Once you’ve opened the page, you’ll need to download a W-9 form, which is a standard IRS form used to provide your Taxpayer Identification Number (TIN). Fill out the form with your information, making sure to include your TIN and sign it. Also, You will need to fill out the new application form where you can find the link below. This will give our staff your information so that we are able to open a new account for you. After you’ve completed these forms, you can then submit it back to Quality Automotive Parts via Email or fax. Once the company has received and processed your forms, we will review your application and open an account for you.
